DOB records show the department issued, then dismissed, 38 elevator-related violations across all five boroughs on October 29, all for the same underlying paperwork lapse: failure to file a 2025 affirmation of correction after a Category 1 five-year test or a routine periodic inspection.

Per DOB Safety Violations data, the violations fall into two types. FTC-VT-CAT1-HA covers missed affirmations tied to Category 1 testing, the five-year full inspection cycle for elevators. FTC-VT-PER-HA covers missed affirmations tied to periodic (annual) inspections. Every record in this batch lists a cycle end date of December 31, 2025, and every one carries a status of "Dismissed."

Addresses affected include 2946 West 23 Street and 2832 West 23 Street in Coney Island-Sea Gate, Brooklyn; 401 West 18 Street, 401 West 16 Street, and 427 West 26 Street in Chelsea, Manhattan; 1065, 1125 and 1135 University Avenue in Highbridge, the Bronx; 65-16 Parsons Boulevard and 67-01 Kissena Boulevard in Pomonok-Electchester-Hillcrest, Queens; and 111 Jersey Street and 125 Cassidy Place in St. George, Staten Island, among others. Several buildings — including the University Avenue cluster in the Bronx and the Kissena Boulevard and Parsons Boulevard addresses in Queens, which share the same BBL and community board — show more than one device cited on the same day.

DOB's violation remarks describe the underlying requirement plainly: after a Cat 1 test or a periodic inspection finds and corrects a defect, the building owner has to file an affirmation that the correction was made. Miss that filing and DOB issues a violation. In this batch, all 38 were dismissed the same day they were logged, per the dataset, with no indication in the record of why.

Separately, DOB's complaint-received data logged 100 new complaints citywide over the same two-day window, including several elevator complaints (category 6S) at addresses not connected to the violations above, among them 1530 Fulton Street in Brooklyn, 455 Schenectady Avenue in Brooklyn, 2185 Valentine Avenue in the Bronx, and 3671 Broadway in Manhattan. Those complaints are unverified at this stage and are not confirmed incidents.
