The first is recorded at 222 Broadway, in the Financial District, BIN 1001245. Per DOB records, the incident happened September 28 and is categorized as "Worker Fell," with one injury and no fatality recorded. The address sits on block 89, lot 12, ZIP 10038, inside Community District 101 and City Council District 1.

The second is recorded at 219 East 42nd Street in Midtown, BIN 1037551. Per DOB records, this one is logged as an "Accident" on September 25, also categorized "Worker Fell," also with one injury and no fatality. That address falls in Community District 106, City Council District 4, ZIP 10017.

DOB has not released cause findings for either incident, and no fault is assigned in the record type or category fields — those describe what happened, not why. Both remain open incident records as of this writing.

A third record filed this week, at 487 Carlton Avenue in Prospect Heights, Brooklyn (BIN 3057520), logged a "Scaffold/Shoring Installations" incident on September 27 with no injury and no fatality reported.

For contractors: two fall incidents at active Manhattan sites in the same week is the kind of pattern DOB inspectors notice, and a site with an open incident record can expect closer scrutiny on its next routine inspection. Anyone with permits active at either address should expect their fall-protection plan to get a second look.
